FAES Insurance: Health ID Card Reissue

FAES was recently made aware the MetLife group number printed back of the Luminare Health ID card was in fact the FAES customer number which is used for administrative plan purposes. The correct MetLife group number is 0322476. We are working with Luminare to reissue ID cards to all plan participants. In the meantime, dental providers can validate coverage using group number 0322476 or the member’s social security number. If plan participants would like a MetLife dental specific ID card, this can be obtained via the MetLife member portal. Information on how to access the MetLife member portal can be found on page 27 and 28 of the FAES Benefit Guide. Please know that all other information printed on the ID card is accurate.
